Coordinated Prevention Network (CPN)

In 2001 the Los Angeles County Department of Health Services (LACDHS) and the U.S. Centers for Disease Control and Prevention (CDC) sponsored local demonstration projects to increase education and testing for the co-morbidities of HIV/AIDS, sexually transmitted diseases, tuberculosis and substance abuse. These aims were to be accomplished through the development of four community-led coalitions of prevention providers. Funding was targeted to specific geographic Service Planning Areas (SPAs) in LAC based on their significant disease burden profile and racial/ethnic community composition. The four lead agencies selected to develop Coordinated Prevention Networks (CPN) were JWCH (SPA 4), Minority AIDS Project (SPA 6), AltaMed Health Services (SPA 7), and the City of Long Beach (SPA 8).

Each lead agency was expected to increase the number of people receiving education about and being tested for the four co-morbidities. Expectations of the four CPNs included membership of at least ten agencies spanning these health issues, offer health education trainings to staff, provide relevant services to clients, conduct referral follow-up, collect and report co-morbidity data for clients receiving testing, collect and report network process and outcome monitoring data and disseminate lessons learned.

CHIPTS provided logistical support, technical assistance, data management expertise, and evaluation guidance to the CPN lead agencies and the local health department.

CHIPTS’ specific objectives included:

  • Develop disease burden profiles for each of the four CPN SPAs and provide technical assistance to lead agencies in the use of the findings
  • Assist lead agencies in developing a standardized data collection tool that would gather client-level data and provide trainings on the utilization of the tool (via Client Intake Form)
  • Develop an instrument to assess the service coverage, linkage, and gaps within the respective CPN (via Network Assessment Survey)
  • Review the four CPN’s scopes of work and their respective evaluation plan and to provide recommendations
  • Conduct quarterly Steering Committee meetings to address CPN program issues along with other regular meetings with lead agencies and OAPP to address data collection and management issues.
  • Provide a set number of hours of technical assistance to the lead agencies
  • Disseminate lessons learned in forms of written reports to OAPP, oral or poster presentations at conferences, developing manuscripts for publications, and hosting a forum to share lessons learned findings
  • Develop and conduct a telephone survey to assess the evaluation needs of Health Education/Risk Reduction, Prevention Case Management, and School-based providers funded by OAPP.
